Health. I certify that my dog is current on all required vaccinations (rabies, DHPP, bordetella) and is healthy to the best of my knowledge. I authorize Nice Diggz to contact my veterinarian or seek emergency veterinary care if needed, and I agree to pay any costs incurred.
Grooming. Nice Diggz is not responsible for complications arising from matted or neglected coats, pre-existing skin conditions, stress-related reactions, or any condition that could not reasonably have been detected before grooming. Additional fees may apply for coats requiring extra preparation.
Daycare & group play. I understand that dog-to-dog interactions carry inherent risks, including minor injuries, illness, or stress despite all precautions. Nice Diggz takes all reasonable care but is not liable for injuries arising from normal dog interactions, illness transmitted despite vaccination requirements, or incidents beyond its reasonable control.
Disclosure. I confirm that I have disclosed all known aggression, bite history, and behavioural concerns in this form. If I knowingly withhold this information and an incident occurs, I accept responsibility for any resulting harm. Nice Diggz reserves the right to refuse or discontinue service to any dog that poses a risk to staff or other animals.
Release.I release, hold harmless, and indemnify Nice Diggz from all claims, damages, and demands arising from my dog’s participation in our services, except in cases of gross negligence by Nice Diggz.
